Substitute Form

Reimbursement for substitutes can only be issued if it is properly recorded.

A separate form should be filled out for each day/time period that you work as a substitute.

Please only fill out the form AFTER you have completed your substitute.

Sub reimbursement will be issued up to two weeks after this form is filled out.
